ID Card Office
The SIU Identification Card Office is responsible for issuing all new and replacement ID cards to students, faculty and staff.
New Faculty and Staff IDs:
New faculty and staff identification cards will be $20.00. The cost will be the responsiblity of the faculty, staff, or campus department.
Campus departments who choose to cover the cost of a new faculty or staff ID (or the cost of replacing a legacy ID), should complete the Department Payment for Faculty/Staff ID Request Form. The completed form will need to accompany the new employee when they visit the ID office to obtain their first ID card.
New employees should bring a copy of their employment contract to obtain the ID card along with a legal form of ID which includes a passport, driver's license, military or state ID. No other forms of identification are accepted.
Replacement IDs (lost, stolen, damaged, or legacy) will be the responsibility of the employee at $30.00. Payment must be received at the time of production, and cannot be charged to a Bursar account.
Department Payment for Faculty/Staff ID Request Form
New Students IDs:
New student identification cards will be $20.00 and charged to the student's Bursar account.
When obtaining an SIU ID Card, students must provide a legal form of identification which includes a passport, driver's license, military or state ID. No other forms of identification are accepted.
Replacement IDs (lost, stolen, damaged, or legacy) will be the responsibility of the student at $30.00. Payment must be received at the time of production, and cannot be charged to a Bursar account.
Current Faculty, Staff, and Student IDs:
Any current faculty, staff, or students may upgrade their legacy ID at the cost of $30.00. Payment must be received at the time of production, and cannot be charged to a Bursar account.

We accept credit, debit, Debit Dawg, and cash.
Checks are not accepted.

The ID Office is located on the second floor of the Student Center across from the Administration Office. Normal business hours are Monday – Friday from 8 a.m. to 4:30 p.m.

In order to better serve the students of Southern Illinois University, the ID Office asks that the students do not post their own materials on bulletin boards, but instead bring no more than two (2) flyers to the ID Office where they will be approved and posted throughout the building.
Information must be approved by the Information Supervisor for the following:
- Pertinence (targets the majority of the student body)
- Date (two week limit)
- No size limit on posters
- Prior material will be displayed as space allows
Failure to check flyers at the ID Office will result in their removal from the bulletin boards.
